  • Overview

    Our Discharge Planner coordinates, conducts and implements discharge planning for inmates at the facility. Maintains effective communication with facility staff and community resources as well as maintain accountability for services provided.

    Qualifications

    Education:

    • Bachelor's degree in social work or closely related field from an accredited college or university

    Experience:

    • Previous experience in developing successful transition plans involving community resources for persons with mental illness
    • Experience in social work case management in a mental health agency for mentally ill population, a must

    Licenses/Certifications:

    • Current CPR certification
    • Any other certifications (such as First Aid) required by contract

    Responsibilities

    1. Responsible for discharge planning for inmates receiving mental health services, particularly inmates diagnosed with a Severe and Persistent Mental Illness (SPMI), Mental Retardation (MR), or inmates with a serious medical illness, in accordance with the policy and procedures.
    2. Responsible for creating and maintaining relationships with community agencies providing services to persons with mental illness or serious medical conditions.
    3. Facilitate applications of eligibility for SSDI, SSI, or other available forms of financial assistance, as well as resumption of benefits for inmates awaiting release from the Facility.
    4. Maintain effective working relationships with Facility Staff, Institutional Parole Officer, and appropriate parole, mental health and medical staff.
    5. Responsible to abide by security regulations, including limits of confidentiality of inmate information requirements of the Facility.
    6. Discharge planning for inmates receiving mental health services or who are seriously medically ill. Types of activities may include but are not limited to:
    7. Initiate contact with inmates who are identified as in need of community mental health care or who are seriously medically ill no later than 120 days prior to release or as soon as is possible.
    8. Identify release-planning needs of inmates receiving mental health services, or with serious medical illness.
    9. Provision of a Life Skills Program to include individual and group activities designed to enhance inmate's coping skills and support upon release.
    10. Coordinate medication and medical needs upon release, including potential residential placement.
    11. Assist inmate in making appointment with mental health agencies for continuation of services.
    12. Communicating with agencies serving those receiving mental health services to develop options for residential placements, including nursing home placements if needed for the serious medically ill.
    13. Communicate and maintain positive working relationship with appropriate Unit Team Staff, Institutional Parole Officers or other facility staff also assigned to release planning activities.
    14. Document services provided to the inmate in the health record, including contacts with community providers or parole staff after inmate's release from custody.
    15. If required by contract, maintain tracking logs of any follow-up contacts provided after inmate's release from facility.
    16. Attend mental health treatment team meetings or medical utilization management meetings to enhance release planning activities and maintain a team approach. Participate in staff meetings and in-service training as provided for Mental Health Services. Attend community based meetings relevant to release planning as directed by the Mental Health Coordinator, Clinical Supervisor or Mental Health Director. Compliance with employee standards of the Facility. Compliance including but is not limited to, the maintaining of a professional working environment and personal appearance consistent with professional responsibility, development of positive working relationships, and timely notification of supervisory personnel of absences from the institution.
    17. Must be able to obtain and maintain security clearance.
    18. Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
    19. Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.
    20. Must be alert at all times; pay close attention to details.
    21. Must be able to work under stress on a regular or continuous basis.
    22. Post orders, if applicable, per site contract.
    23. Perform other duties as assigned.
