  • Overview

    Our Re-entry Specialist works with patients to identify resources that they can connect to when they return to their communities. The Re-entry Specialist will establish relationships with providers of mental health and substance use disorder services, housing providers, and advocates in each of the communities that the program serves. He/she will work most closely with individuals who are close to becoming competent and who are likely to have relatively quick disposition of their case so that they may soon be returning to the community. At the jail, the Re-entry Specialist will provide individual and group treatments that help the patient to establish the skills and supports needed for discharge. .

    Qualifications

    Education:

    • Bachelor’s degree from an accredited college or university required.
      Master’s degree from an accredited university in a human services field preferred.

    Experience:

    • One (1) year experience working with mental health or criminal justice populations.
    • A high level of familiarity with the medical and mental health systems at the local, state, and federal levels is desirable.
    • Excellent interpersonal skills. Must be able to communicate effectively with all levels within the facility, in addition to a variety of outside customers with varying interests.
    • Must be able to thoroughly and effectively document all work performed in the position.
    • Ability to work with computers and the necessary software typically used by the department.
    • Must display a high degree of empathetic listening, tact and kindness toward others.
    • Excellent written, verbal, and computer skills.

    Licenses/Certifications:

    • None required.

    Responsibilities

    1. Serves as a liaison between hospital and community, family and significant others to maximize the patient’s ability to be successful in the community after discharge. 
    2. Identifies resources that the patient can connect to after disposition of his case.
    3. Visits and phones mental health and substance use disorder services, housing providers, and advocates in each of the communities that the program serves.
    4. Provides individual and group treatment that helps the patient to establish the skills and supports needed for discharge.
    5. Participates in discharge meetings.
    6. Participates in Stakeholder meetings.
    7. Establish and maintain contact with patient’s legal representation to facilitate discharge planning and support competency treatment as needed.
    8. Develop comprehensive discharge packet to include summary of treatment, community resources, and other relevant information need for patient. 
    9. Completes all documentation accurately and timely.
    10. Demonstrates cultural competency in all aspects of duties performed and particularly evident in interactions with and treatment of patients.
    11. Attends and successfully completes competency training on an annual basis.
    12. Demonstrates age specific competency in all aspects of duties performed and particularly evident in interactions with and treatment of patients.
    13. Is honest, consistent, positive, and considerate in day to day interactions.
    14. Cooperates with colleagues.
    15. Attends weekly department meetings and participates actively in problem solving and idea sharing within the department.
    16. Must be able to obtain and maintain security clearance.
    17. Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
    18. Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.
    19. Must be alert at all times; pay close attention to details. 
    20. Must be able to work under stress on a regular or continuous basis.
    21. Post orders, if applicable, per site contract. 
    22. Perform other duties as assigned.
